These authors suggest that if cross-modal plasticity restores metabolism in the auditory cortex before implantation, the auditory cortex may no longer be as able to respond to signals from a cochlear implant, and prelingually deaf patients will show no improvement in hearing function (despite concentrated rehabilitation) after implantation of a cochlear prosthesis. We hypothesize that specific strategies will be needed to modulate brain processing and to enhance the extraction of relevant and functionally meaningful information generated from neuroprosthetic inputs. The results of these studies have important implications.

    First, they suggest that functional neuroimaging may have a prognostic value in selecting those individuals who are ideal candidates for a cochlear implant. It is conceivable that a corollary scenario exists regarding cross-modal sensory interactions within the visual cortex of the blind, and we therefore suspect that such a combined approach (i.e., using information obtained by neuroimaging) may help identify candidates who are most likely to succeed with a visual prosthesis implant. (1998) found that in prelingually deaf patients (i.e., hearing loss prior to the development of language), the primary auditory cortex was activated by the sound of spoken words following receipt of a cochlear implant device. This group found that the degree of resting hypometabolism before device implantation was positively correlated with the amount of improvement in hearing capability after the operation, even more astounding.
